Plans and Summary Plan Description
Baseball Players Benefit Plan, your benefit will be two times your annual earnings, minus
the amount of your Major League benefit, up to $80,000 when combined with the Major
League Baseball Players Benefit Plan. If you are covered under the Minor League Health
and Welfare Plan, your benefit under this plan is two times your annual salary minus
$20,000, up to a maximum of $80,000 when combined with the Minor League benefit.
Your spouse will also have coverage in the amount of $5,000 and each of your children
will have coverage in the amount of $2,000.
You may be able to purchase additional coverage for yourself (subject to any
underwriting or other proof of insurability requirements) in multiples of $10,000 (up to
$300,000). Subject to policy terms, you can also purchase additional coverage for your
spouse in multiples of $5,000 (up to $150,000) and for your children in the amount of
$10,000.
The Plan also provided for benefits in the event of a loss in connection with an accidental
death or injury (“AD&D”). Your AD&D benefit is equal to your Life Insurance benefit.
You may apply to purchase additional AD&D coverage up to the amounts of any
additional Life Insurance benefit.
Your coverage will be reduced once you reach age 65, until at age 75 your benefit will be
35%. You have a limited right to convert the coverage to an individual policy if your
coverage under this group plan terminates.

LONG TERM DISABILITY INSURANCE BENEFITS
ELIGIBILITY TO PARTICIPATE
Eligible Employees: All active L.L.C. owner-employees or non-seasonal employees of
the Nationals regularly working a minimum of 35 hours per week who are citizens or
residents of the United States, and all active non-seasonal coaching or training employees
working a minimum of 35 hours per week who are foreign nationals under contract with
the Nationals to perform services in the United States (“Full-Time Employees”).
Insurance Effective Date: February 1, 2015
New Hire Effective Date: Coverage will become effective on the first of the month
coinciding with, or next following, 30 consecutive days of regular Full-Time
employment.
CESSATION OF PARTICIPATION
Your coverage will end on the date on which you terminate employment.
