The Washington Nationals have contracted with Standard Insurance Company provide life
insurance benefits, long term disability benefits, vision benefits and claims services under the
Plan. Benefits are paid entirely by Standard Insurance Company in accordance with the terms
of the Plan and are guaranteed under the contract.
No action at law or in equity may be brought until 60 days after you have submitted proof of
loss. No such action may be brought more than three years after the earlier of:
1. The date the Standard receives proof of loss; and
2. The time within which proof of loss is required to be given.
