SHORT TERM DISABILITY BENEFITS
ELIGIBILITY TO PARTICIPATE
Eligible Employees: All active L.L.C. owner-employees or non-seasonal employees of
the Nationals regularly working a minimum of 35 hours per week who are citizens or
residents of the United States or Canada, and all active non-seasonal coaching or training
employees working a minimum of 35 hours per week who are foreign nationals under
contract with the Nationals to perform services in the United States (“Full-Time
Employees”).
Insurance Effective Date: January 1, 2014, as amended January 1, 2015
New Hire Effective Date: Coverage will become effective on the first of the month
coinciding with, or next following, 30 consecutive days of regular Full-Time
employment.
CESSATION OF PARTICIPATION
Your coverage ends automatically on the earliest of:
1. The date the last period ends for which a contribution was made for your coverage.
2. The date the Plan terminates.
3. The date your employment terminates.
4. The date you cease to be an eligible employee.
ELECTIONS AND CONTRIBUTIONS
You are not required to contribute towards the cost of this coverage.
BENEFITS
This program pays 70% of the first $4,807 of your weekly pre-disability earnings
(minimum weekly benefit of $15; maximum weekly benefit of $3,365 before reductions)
once you have been unable to work in your own occupation due to a disability for a
period of 7 days. You are considered disabled if you are unable to perform the material
duties of your job and you suffer a loss of at least 20% of your pre-disability income.
The maximum benefit period is 90 days. Benefits will be reduced to the extent you earn
certain other income while disabled.
