Case #2:
Causation: Breaches in safety protocol and procedures
The training event was an officer down / officer rescue scenario presented by a large state police training
bureau. It was open to officers and deputies from agencies throughout the state.
It is important to note that not all police officers receive force-on-force training, therefore the officer had
no duty to be knowledgable of the safety protocol for force-on-force training. She was reliant on the
Instructor staff to provide a safe environment. This was a professional law enforcement training event.
• On this date, and at this event, there was an insufficient supply of safety equipment so officers use of the
industry standard mandatory full head, face and groin protection specifically designed for use in force-
on-force training was optional.
