                                              Policies and Procedures
            4.4     Position: Clinical Director

             Summary: The Intensive Case Management (ICM)/Independent Care Waiver Program (ICWP) Clinical Director
             provides clinical oversight concerning direction, strategic planning and coordination of service delivery for
             individuals those clinicians under their management.
             In collaboration with Program Directors, coordinates department resources in an efficient and economic
             manner to enable each individual to attain or maintain the highest practical physical, mental, and psychosocial
             well-being. Collaborates with other departments, medical professionals, consultants, and organizations,
             including state agencies and advocacy groups, to develop, support and coordinate individual care, related
             administrative functions, and to represent the interests of the organization and those served. They provide
             input in the coordination and improvement of services and work with provider agencies to insure timely and
             efficient service delivery. In addition, the Clinical Director may be asked to serve on task forces/committees and
             represent Crew Consulting Services LLC to the public. Concurrently, they interact with other agencies, provide
             information to and respond to requests from individuals and their families, community representatives, State
             offices, Providers and the general public which involves explaining programs and policies, handling complaints
             and grievances and speaking before the public and groups. The position works closely with Director to
             implement a shared vision, advance projects, and establish priorities within the various Care Coordination
             programs.

             4.5    Staffing Issues

             Any vacancies will result in other case managers covering cases for 60 days or less while a replacement is being
             hired and trained. During vacations, case managers will be required to conduct billable services for the month for
             a pre-designated number of individuals. The case manager ’s supervisor will be responsible to attend to any
             issues that arise during the case manager’s temporary absence.
             5. Training Plan

             5.1    CCSP- Community Care Services Program Waiver and SOURCE- Service Options Using

                    Resources in A Community Environment
             Prior to working with individuals, all case managers and supervisors will participate in privacy education within 30
             days of hire. Within 60 days of hire, all case managers and supervisors will complete the Department of Community
             Health’s (DCH) Case Management Certification Training.

             CCS will also provide training in Prevention of Abuse, Neglect & Exploitation Practicum. All case managers
             and supervisors will also complete a provider-developed orientation that includes, but is not limited to:

                  Overview of the Agency
                  Mission, philosophy, goals, services, and practices
                  Personnel policies

                  Training in Health & Safety

                  Understanding Service Plans & Individualizing services
                  Implementing policies and procedures

                  Who can act on behalf of the individual?
                  Provision of the Notice of Privacy Practices
