                                              Policies and Procedures
             1. Mission Statement

             The founding mission of Crew Consulting Services LLC (CCS) is to make a difference in the lives of the ageing and
             disabled. We will raise the standard of care through quality assurance and compliance education. The focus
             is on the individual and to ensure that all providers can provide the best quality care to the individuals that
             they serve. The goal is to consult providers in maintaining quality care for individuals while maintaining the
             dignity and privacy of the individuals and families served.

             CCS is a care coordination and consulting-focused company, helping people understand the choices they have
             in the community and select the best options for the individual. CCS does not believe a checklist represents a
             person, and we deeply believe people need an advocate who is available and accessible, that knows them or
             their family member, and really knows how to listen, address their concerns and provide them with choices
             and the opportunity to be happy. We want to help guide people in their journey.

             2. Organizational Governance

             Crew Consulting Services LLC(CCS) operates under the guidance of its Chief Executive Officer/Owner. The names
             of the owner as well as their affiliations, and any potential conflicts of interest are available upon request.

             All positions described and information included in this Policy and Procedure Manual are pursuant to compliance
             with the Supports Program (SP) and Community Care Program (CCP) Policy & Procedure Manuals and applicable
             Division Circulars; policies and procedures related to reporting Medicaid waste/fraud/abuse; Protected Health
             Information (PHI) - Health Information Portability and Accountability Act (HIPAA); human rights; emergencies (and
             how they will be dealt with); reporting unusual incidents; personnel; and first aid training and CPR requirements.

             It is agreed amongst all contractors of CCS, in accordance with the Department of Community Heath, to
             assure the opportunity for all individuals to receive quality services and supports, participate meaningfully in
             their communities and exercise their right to make choices. The chart below outlines CCS’ organizational
             governance to assure effective and ethical management of the operations of the company.
