• Healthcare Sanctions Check: is a tool for providing insight into a healthcare professional’s medical
background, searching for penalties, suspensions, or punitive or disciplinary actions taken against a
healthcare professional.
• Healthcare Statewide Criminal Search: is a state law enforcement agency repository which typically
contains any misdemeanors, felonies committed by the applicant.
• National Sex Offender Registry: reviews all 50 states for those who were required to be listed as a sex
offender.
• Widescreen Plus National Criminal Search: is a tool to check numerous, diverse, and unique sources to
help uncover an applicant’s criminal history in jurisdictions beyond an applicant’s known address history.
• Professional License Verification: confirms a variety of information such as the licensee’s name, if the
license is active and in good standing with that board, the issue and expiration date and it the board
has sanctioned or restricted the license.
Fingerprinting
The following additional background searches will be required if applicable to the position:
• Motor Vehicle Records: provides a report on an individual's driving history in the state requested. This
search will be run when driving is an essential requirement of the position.
3.3 Leave Time and Holidays
CCS does not currently offer Paid Time Off to our independent contractors.

3.4 Overview of Company Benefits
CCS does not currently offer a comprehensive benefits package to our independent contractors. Reimbursements
Auto Allowance: CCS’ offers a mileage rate to all independent contractors based on job assignment.
3.5 Professional Conduct
CCS contractors are an example to peers, individuals, and vendors. Contractors are expected to conduct
themselves with high professional standards of conduct while performing their duties. This is to include treating
others respectfully and observing all laws, respecting company property, and utilizing company property for
business purposes only.
Our policy forbids accepting any gratuity from individuals, family members, or vendors with which we do
business. Gratuities will be returned with a letter of explanation of company policy.
