of the anniversary year. The company reserves the right to create blackout periods during which PTO should be
avoided if possible and during which requests will be subject to close review by management.
II. Accrual Schedule
The annual PTO accrual is determined by years of service and job level, and the increase in benefit is effective the pay
period following the 2, 5 and 10 year anniversary date.
Years of service is defined as the cumulative number of years employed at A-MAX Auto Insurance. Refer to Rehires
section III for more information.
The following Accrual Schedule is based on a 40-hour work week.

III. PTO and Employment Status Changes
Termination
• In the event of termination, including resignation or any other event other than involuntary lay-off, the value of
the unused PTO balance (PTO accrued less PTO taken) will not be paid in full. If there is a negative balance, that
value will be owed to A-MAX Auto Insurance and will be deducted from the final paycheck, in accordance with
applicable laws.
• In the event of involuntary lay-off, the value of the unused PTO balance (PTO accrued less PTO taken) will be
paid in full. If there is a negative balance, the employee is not required to pay back the balance to A-MAX Auto
Insurance.
Leave of Absence
In the event of a Leave of Absence, employees may use accrued PTO to supplement income during a leave. Note, there
is no accrual of PTO while on leave of absence. PTO accrual will continue following first pay period following return to
work date.
