by the end of the anniversary year. The company reserves the right to create blackout periods during which PTO
requests should be avoided if possible and during which requests will be subject to close review by management.
II. Accrual Schedule
Refer to the Accrual Schedule below as accrued PTO hours are based on the number of eligible hours worked during each
pay period. The annual PTO accrual is determined by years of service, and the increase in benefit is effective the pay
period following the 1, 3, and 5 year anniversary date.
Years of service is defined as the cumulative number of years employed at A-MAX Auto Insurance. Refer to Rehires in
section III for more information.

III. PTO and Employment Status Changes
Termination
• In the event of termination, including resignation or any other event other than involuntary lay-off, the value of
the unused PTO balance (PTO accrued less PTO taken) will not be paid. If there is a negative balance, that value
will be owed to A-MAX Auto Insurance and will be deducted from the final paycheck, in accordance with
applicable laws.
• In the event of involuntary lay-off, the value of the unused PTO balance (PTO accrued less PTO taken) will be
paid in full. If there is a negative balance, the employee is not required to pay back the balance to A-MAX Auto
Insurance.
Leave of Absence
In the event of a Leave of Absence, employees may use accrued PTO to supplement income during a leave. Note, there
is no accrual of PTO while on leave of absence. PTO accrual will continue following the first pay period following return
to work date.
Family and Medical Leave Act
The Family and Medical Leave Act (FMLA) provides eligible employees with time off for certain qualifying leaves.
Qualification for PTO benefits shall have no effect on an employee’s qualification for FMLA leave. Employees may qualify
for FMLA leave regardless of their qualification for PTO benefits. For information about eligibility requirements for FMLA
unpaid leave, please see your Human Resource Representative.
