Recommendations
1. Tanker's overall checking system should be strengthened to ensure thorough healthiness of the tankers
before sending the tankers for loading at Gantry.
2. Proposed to carryout checking of Benzene tanker by surveyor for Domestic Benzene tanker's so that before
entry in PNC gantry, it is checked for any abnormality in-line with the practice is being followed for Benzene
export tankers (copy of WO enclosed) and also of MEG/DEG/TEG and Butadiene.
3. Though the tankers are checked once in three month and green cards are being issued by PMC, there could
chances of changes/modifications in the vehicle by the transporter (within the green card validity period)
which can lead to similar incidents in future.
Case 11 Date: 10.09.2019 Time: 18:30 hrs.
Incident
In NCU, Fit-up of Radiant Coils (vertical coils) of Heater-500 was in progress by M/s-Radiant Engg. team. Assembly
of five coils (SN-I/L 1 to I/L 5) with approx. 15m length was hanged with the help of chain pulley block. Horizontal
clamps provided in this vertical coil assembly for lifting. During repositioning for fit-up by guide rope; suddenly
horizontal clamp holding coils assembly slipped & coil assembly fell at ground from a height of
approx.1.5 to 2 meters.

Root Cause
It was found that horizontal clamp provided for lifting of assembly wasn't properly fixed.
Recommendations
1. Clamp of proper design to be used and Tightening of Horizontal clamp to be ensured before erection of
radiant coil assembly.
2. Apart from the clamp, additional reinforcement to be provided at the bottom of the assembly and same shall
be tied to the clamp /lifting assembly as additional protection.
3. Area underneath to be barricaded while lifting/fixing the pipe assembly.
