Recommendations
1. Proper SOP and Work instruction in bilingual to be developed and displayed at the sample bottle washing
room covering details when & who to remove tags, segregation and cleaning of sample bottles as per their
chemical nature.
2. Ensuring proper training to samplers for washing all types of chemical bottles and training record of the same
to be maintained.
3. Proper job allocation to be ensured by supervisors.
4. All the MSDS of all chemical related to QC must be available in QC lab.
5. There must be an SOP of handle/dispose of all samples/chemical which are being handled in QC lab.
Case 15 Date: 17.02.2020 Time: 10:10 hrs.
Incident
On dated 17.02.2020 at around 10:10 am shifting of low pressure Nitrogen cylinder activity was being carried
out in Store no 6. While shifting one empty nitrogen cylinder with lock pin suddenly operated and fell down
on ground surface and leakage of N2 gas started which resulted to falling of other cylinders of nearby area.
N2 valve of other empty cylinder got damaged due to falling on ground surface. Workers handling cylinder
barely escaped from site & immediately called their supervisor.
Root Cause
Accidently actuation of cylinder valve due to dislodge of lock pin.
Recommendations
1. SOP to be prepared for Fixing /Loading/Unloading N2 cylinders of DCP Tender.
2. Proper storage facility (rack with chain arrangement) shall be provided for storage of filled/empty Nitrogen
cylinders of DCP Tender.
3. Low pressure Nitrogen cylinder shall be emptied out through system at DCP Tender itself before unloading.
4. Loading and unloading of N2 cylinders are to be carried out in presence of Shift officer (F&S) /JEAs on duty.
5. Cylinder caps to be remained attached on the cylinders during loading/unloading/transportation of N2 cylinders.
6. Healthiness of cylinder valves along with locking pins is to be physically checked after every refilling from
vendor's workshop.
