Case 12 Date: 11.09.2019 Time: 05:45 hrs.

Incident

Jeep driver named Mr. Pradeep Rawal driving Bolero Jeep from MEG towards SWING hit a car (HR-06-AN-7680
Maruti Swift) parked near swing unit (SRR-4). The incident happened due to sleepiness of the Bolero Jeep driver.
Minor damage in both cars reported.

Root Cause

1. Driver felt sleepiness since, he was driving the vehicle continuously from 5-6 days round the clock during
M&I shutdown.
2. Driver was over speeding the vehicle at the time of incident.

Recommendations

1. For monitoring the duty Hrs. , provision in Access control system (Punch card system through boom barrier)
to be made for the contract driver in line with existing system. Driver change over duty to be monitored and
ensured at every 12 hrs.

2. Fixed speed monitoring systems to be installed at selected locations.

Case 13 Date: 03.10.2019 Time: 12:30 hrs.

Incident

In NCU, near heater-300 backside area in pipe rack, flash over occurred while carrying out cable coupler
joint for welding machine.
