Case 18 Date: 27.07.2020 Time: 12:00 hrs.
Incident
Field in-charge, Sh. Chandra Prakash (PNM) was on normal plant round in NCU. Suddenly one tin sheet of CGC
shed fallen from the height of almost 50-60 ft just near him when he was in CGC Compressor area.
Root Cause
1. Loosening of rivets due to wind over a period of time.
2. Corrosion over a period of time and detachment of percolated roof sheet due to heavy wind.
3. Identification of corroded loose sheet went un-noticed.
Recommendations
1. Loose hanging corroded sheet of CGC is to be removed.
2. Corroded and hanging roof sheet to be replaced all across the PNC.
Case 19 Date: 08.07.2020 Time: 10:00 hrs.
Incident
Contractor employee (Satish Electrician) engaged in elect maint of NCU observed spark with sound near the
terminal of outdoor type capacitor bank reactor installed at back of S/S#1. He immediately raised a fire call by
activating MCP NO. 101-L11-127 & 101-L5-10 near Substation # 1 (NCU). Immediately 1st turnout with crew
rushed to the spot. On arrival there was no fire observed in the capacitor bank#1 - 1204 KVAR (6.6KV). Some
motors of AU had tripped during the incident and were restarted immediately.
