Failure of terminal insulator of reactor Failure Point

Root Cause

1. Failure of terminal insulator of reactors of Cap bank#1.
2. Deterioration of insulation and failure of reactor terminal insulator.

Recommendations

1. The frequency of inspection of terminal insulators, connections and checking of I.R value of outdoor capacitor
banks and reactors is to be increased to Yearly.

2. The damaged terminal insulator is to be replaced :

2.1 Inspection of condition of all terminal connections, insulation resistance value of all outdoor type Capacitor
Banks and reactors is to be carried out and recorded immediately.
2.2 Development of SOP/PM for schedule for checking of capacitors.

Case 20 Date: 14.10.2020 Time: 11:10 hrs.

Incident

Fire water network flushing activity was in progress and Diesel pump 26-PM-108 got started on auto as per
logic at 10:40 hrs and was running normal as observed by JEA for 30 mins. Suddenly after 30 mins of operation
JEA of FWPH Sh. Gurjeet noticed smoke coming out from 26-PM-108. He immediately informed to Utility Shift
In-charge on Walkie-Talkie and rushed towards the pump. He found the Diesel Engine body cracked and lube
oil spread on the floor. He also found some internal metal parts of Diesel Engine on the floor. He immediately
pulled out control fuse & put the local control switch on OFF position. Finally he closed discharge valve of the
above mentioned pump and of Diesel line.
