Incident

Four lady workers were being transported on tractor- trolley from tank farm area to fabrication yard. On
road -C Hydraulic jack of trolley suddenly lifted up and one of the ladies fell down on the road. She
received lacerated wound at backside of head as well as tenderness in left wrist joint. She was immediately
given first aid and released.

up position. By not keeping locking nut near lever,
accidental movement of lever towards upside
cannot be avoided.

Root Cause

Accidental actuation of hydraulic lever in the absence of locking arrangement.

Recommendations

Transportation of manpower through Tractor, Trolleys, Open Jeeps, Trucks or any goods transport
vehicles inside PRPC to be stopped.

2. Ensure all locking arrangements (Disengagement of hydraulic pump, Hydraulic lever lock, & trolley lock)
are in place for all such trolleys under transportation inside PRPC.

3. Trolley covers from all side with lock to be ensured before taking it in service.
4. Use of loose clothing by the drivers to be avoided for such accidental actuation
