Root Cause
Improper laying & dressing of power cable & Inadequate supervision during erection of pipe
Recommendations
1. Before erection of new facility, surrounding area to be checked and made clear of any cables, materials,
scraps etc.
2. For carrying out critical erection jobs, job safety analysis shall be carried out to find the possible hazards
involved in the job and their counter safety measures to be adopted.
3. Cables shall not be laid in areas where piping erection is to be done. All HT electrical cables laid in
Prime-G to be dressed up properly.
4. Separate work permits to be issued / obtained for specific job.

Incident
During unloading of acetic acid from the Tanker, driver received superficial burn injury at back and shoulder
due to spillage of acetic acid on him during fitting of metallic hose in tanker valve assembly. He was
administered First aid at First aid centre and release thereafter
