             Root Cause

           1.  Inadequate thickness of trench slab.
            2.  Trench was not adequately filled with sand which was one of the reasons for breakage of the slab.



             Recommendations

           1. Ensure deployment of experienced rigger for such rigging operations.
            2.  Use of Polyester web slings with valid test certificate to be ensured while lifting the Structural members

            3.  Work Permit shall be supplemented with Job safety analysis for critical erection jobs and recommendation
              thereof should be complied.
            4.  Area under the suspended load shall be barricaded while performing lifting & hoisting activities.

            5.  Continuous supervision to be ensured during such Critical (Lifting & Hoisting) activities.

             Incident


            Replacement of Reformer overhead shed in HGU-06 was going on to facilitate catalyst loading with the help
            of Man lift crane. During exit through the cable trench on its pathway, the tyre of the man lift crane slumped
            on the concrete slab leading to imbalance of the crane. However, no further consequences occurred and man
            lift got out of the site safely.
