Recommendations

1. All damaged slabs to be immediately replaced with proper one.

2. All cable trenches to be properly sand filled and proper size of slabs to be put.

Case 8 Date: 19.09.2019 Time: 14:00 hrs.

Incident

While coming up from one of the platforms in Ejector Condensate receiver circuit for assessment of line
replacement job, the middle portion of platform (Chequered plate) got collapsed & both legs up to THE waist
of one of the contract employees got hanged with his full body harness from the broken portion at a height of
approx. 15 mtrs from the ground. Subsequently, he was rescued by his co-workers present in that area safely.

Root Cause

Corrosion on chequered plate due to exposure of steam from the bottom & rain water.

Recommendations

1. Damaged chequered plate platform leading to the incident to be immediately replaced with grating
type platform.

2. Platforms provided with chequered plate in the other areas of the unit to be replaced with iron gratings.

3. All units to carry out a survey for identification of such corroded chequered plates platform and action
plan to be made for replacement of the same.

Case 9 Date: 22.09.2019 Time: 16:00 hrs.

Incident

Tag job related hot work was going on at DCU CDSP 27 M platform near R-104 Ring 'O' valve for steam line
isolation valve replacement. During grinding job sparks fell on the blow down valve blinded flange which led to
fire.
