Damaged Cable
Root Cause
Damage of cable while excavation & non adherence to work permit system.
Recommendations
1. Manual excavation shall be done at designated locations on the route before starting mechanized
excavation.
2. Excavation permit is must from concerned departments as per procedure within battery limit and
township.
3. Lying of cable must be as per IE rule & IOCL standard.
4. For Outside Battery limit, P&U is to issue a drawing showing cable route so that excavation can be done
along the said route.
5. Any abnormality while working like cable damage in this case, that is to be reported immediately to site
in charge / owner in charge.

Incident
During erection of a 10“ pipe in main pipe rack of Prime-G using hydra & chain pulley block, an armored
cable got crumpled in between pipe and concrete beam of pipe rack causing spark.
