Root Cause

1. Entry of excess fuel through fuel pump/ injector in combustion chamber.

2. Malfunctioning of Fuel Injection pump due to failure of delivery valve/ increase in gap of plunger leading to
dribbling action and direct fuel supply to piston cylinder. Due to that excess heat generated causing melting
pistons head & their seizure OR.

3. Fuel Injector stud/ pin has got stuck leading to uncontrolled fuel supply to piston cylinder causing excess
generation of heat and melting of pistons head & their seizure. (as per MOM dtd. 23-Oct-20 with M/s Kamboj
Power Generator Service enclosed as Annexure-3).

4. Major overhauling of Diesel Engine is required.

Recommendations

1. Major overhauling of all Diesel Engines to be carried out.
2. Pricol Panel of all Diesel Engines to be upgraded with updated version to include additional alarms for

better analysis.

Case 21 Date: 20.10.2020 Time: 06:01 hrs.

Incident

PNC fire control room received a telephonic message from HDPE regarding smoke coming out from one of the
CFL fixture installed in HDPE extruder PLC room. Fire tender with crew immediately rushed to the spot &
observed smoldering in one of CFL fixture installed in HDPE Extruder PLC room. Immediately information was
given to utility SIC. Utility and CO2 fire extinguisher applied to quenching the smoldering. SIC visited the site
& found ELCB trip. Crew returned back to Fire station.
