• Start with all personal details and a
statement of intent. Pregnancy and
previous birth experience.
• Add a note that this is your
preference all being well, but you
appreciate that if birth takes a
different path you are totally
supportive of your medical team and
will listen to any suggestions they
make for your care.
• Then use visual birth symbols to
communicate what is most
important to you. Add them in order
of greatest importance, so that the
ones you care most about take
precedence.
• It should be a one-page, at-a-glance
synopsis for your caregivers to get to
know you and your wishes quickly.
