Facility

A facility survey or an “inspection” is a periodic review that is conducted in person by surveyors from the Department of

Health, Department of Human Services or Department of Aging. These surveys are typically conducted yearly but could be

conducted more often in response to complaints. Ombudsman are involved in facility surveys by providing surveyors with

general information about the types f complaints they have received. When the facility survey s completed an exit,

i

meeting s conducted at the meeting to review the surveyor’s findings. Ombudsmen can attend these exit meetings.

During your training you will shadow a Facility Survey activity. While shadowing that activity, complete the Trainee Journal

for Facility Survey.

There s also a Pre-Survey Report that you can fill out prior to attending a survey exit meeting. This will help you compile

information about the facility to share with the surveyors prior to the survey.
