A care plan is a required person-centered “plan of care” in nursing homes which documents how the facility plans on

helping the resident meet needs and achieve goals. One f ombudsmen’s primary roles regarding Care Plans is to educate

residents about their purpose and their right to participate in that process.

During your training you will shadow a Care Planning activity. While shadowing that activity, complete the Trainee Journal

for Care Planning.
