F. Role-play
Work with a partner.
Student A: play the part of a doctor. Take a history and ask related questions according to
the patient’s form.
Student B: play the part of the patient and complete the following form.
Note: Patients often fill in a form similar to the following before their first visit to a doctor.
The doctor should not have to ask questions about all of the different illnesses, conditions
and medical history. S/he may, however, need to ask some of the following questions:
1. I see you’ve had hepatitis. When did you have it? Do you still have any
problems?
2. I see that you’ve had recurrent headaches. Can you tell me a little more about
that?
