Medical Plans
• If you are currently enrolled in the HRA, HSA or MyHealth HRA, you will be moved
to the MyHealth HSA. If you are currently enrolled in the Scott & White Plan, you
must log in to MyWorkday and make your elections.
• There will be no out-of-network benefits in the MyHealth HSA Plan.
• If you use out-of-network providers, you will pay 100% of the expense.
Generic, preventive maintenance drugs will be covered at 100%.
Prescription Drugs
Tobacco User Surcharge If you and/or your covered spouse currently use tobacco or nicotine products,
you must each quit or enroll in our company-paid BlueCross BlueShield tobacco
cessation program to avoid the 2017 monthly surcharge of $75 per person per
month.
HSA Contributions The maximum HSA contribution amount for 2017 increases to $3,400 for individual
coverage and remains at $6,750 for family coverage. To take full advantage of the
HSA, you may need to increase your individual contribution amount. If you make no
changes, your HSA contributions will remain at 2016 levels.
MyHealth Activities In addition to annual physicals, covered spouses will be eligible for company HSA
funding if they complete applicable recommended age and gender screenings.
Working Spouse The surcharge for covering a spouse eligible for medical coverage through his/her
Surcharge employer is increasing to $250 per month.
Vision • Retinal screening photography will be covered with a $10 copay.
• A second eye exam in 2017 will be covered with a $10 copay.
Life Insurance • If you make an election or change to your optional life coverage that requires
Evidence of Insurability (EOI), you will only be required to complete a short form
for the EOI process.
• For employees who retire after December 31, 2016, retiree life insurance will not
be available.
How to Enroll
1. Make your elections for 2017 any time from
October 31 to November 11, 2016 through MyWorkday.
• Call 844-4MY-WKDY (select option ‘7’) for questions.
2. Print your confirmation statement as soon as you complete
your online enrollment. You will not receive a confirmation statement via mail.
