MYHEALTH HSA PLAN                  Tobacco User Surcharge
                                        What’s the single most important step you can take to protect
                                        your health? Quit using tobacco. Tobacco use is a leading
                                        preventable cause of disease, disability and death in the United
                                        States. With this in mind, we’re adding a new tobacco user
                                        surcharge for 2017 to encourage current users to take steps to
                                        get healthier. This new tobacco user surcharge will be $75 per
                                        person, per month (employee and/or covered spouse) and will
                                        apply to the company’s medical plan premiums in 2017.


                                        Commit to Quit Now, Save Tomorrow
                                        In addition to the dramatic health improvements that come with
                                        quitting tobacco, you can avoid the tobacco user surcharge on
                                        your 2017 medical plan premiums by taking action to quit.
                                        •  If you and/or your spouse currently use tobacco products,
                                          you must each quit or enroll in our company-paid BlueCross
                                          BlueShield tobacco cessation program to avoid the 2017
                                          surcharge.

                                        •  If you enroll in the program, you must successfully complete
                                          four coaching sessions by December 15, 2016. By completing
                                          the program, you meet the conditions for avoiding the 2017
                                          tobacco user surcharge.
                                        •  Employees must verify their own and their covered spouses
                                          tobacco-use status during this fall’s benefits open enrollment
                                          process through MyWorkday.



















          Ready to Quit? Company-Paid Program Can Help
          If you’re a tobacco user who wants to quit, call BCBS toll-free at 1-800-762-8532 and ask for Lifestyle
          Management. Enroll now to ensure you have time to complete four sessions by December 15, 2016 to
          avoid the 2017 surcharge.

          The program includes:
          •  Personal Wellness Coach, who is available by phone to look at your lifestyle and habits and help you
            figure out what you need to be successful.
          •  Medication and nicotine replacement therapy guidance.
          •  Get Started toolkit, including “The Little Book of Quitting,” a quit smoking CD and a “Pocket Pal to Quit
            Smoking Without Gaining Weight.”
          •  Ongoing calls and support, available by phone Monday-Friday, 8 a.m.-6:30 p.m. Central.
          •  Online resources, which let you work at your own pace to reach your goals.

          Since coaching session frequency can vary depending on your needs, sign up now to avoid the 2017
          surcharge.
