teacher who issued the grade of “INC” must receive and evaluate the
completion requirement submitted by the student and give a final grade. To
give a final grade, the teacher must accomplish and submit four (4) copies of
the University prescribed Completion Form and have it approved by the
Department Chairperson and College Dean. The teacher submits the
approved form to the College Registrar who will submit it to the University
Registrar. Upon approval by the University Registrar one copy will be retained
by the University Registrar, one copy will be returned to the student, one copy
to the Department Chairperson, and one copy to the College Registrar.”
Removal examination for grades of “4.0”
“Students with grade of 4.0 may take a removal examination as one
final chance to pass the subject after payment of a removal examination fee.
The teacher who issued the grade of 4.0 must give a removal examination to
the students within the schedule prescribed by the University Registrar, check
the test paper, return the test paper to the student, and give a final grade to
the student which may be a failing or passing grade. The teacher must
accomplish four copies of the University Registrar’s prescribed Removal of
Grades Form and submit it for approval to the Department Chairperson,
College Dean, and University Registrar. Upon approval, the University
Registrar returns one copy to the student, one copy to the Department
Chairperson and one copy to the College Registrar.”
