Preparation and submission of grading sheets

“Upon approval of the grading sheets by the Department Chairperson

and College Dean, the teacher must upload the grades of the students at the

University Registrar on or before the prescribed deadline.”

Uploading grades

“Upon approval of the grading sheets by the Department Chairperson

and College Dean, the teacher must upload the grades of the students at the

University Registrar on or before the prescribed deadline.”

Changing of uploaded grades

“The teacher is allowed to change the grades of students that have

been uploaded due to the following reasons:

 Mistake was committed while computing the final grade of the students

and needs to be rectifies;

 Mistake was made during the final printing of the grading sheets; and

 Mistake was made during the uploading of the grades of the students.

Changing of grades must be done by the teacher using the Change of

Grade Form prescribed by the University Registrar.”

Completion of “INC” grades

“A student with a grade of “INC” or incomplete may submit completion

requirements to the teacher who issued the grade within the schedule

prescribed by the University Registrar after payment of a completion fee. The
