5. Less important factors: full bladder, rectum, placenta previa, pelvic
tumor & large head.
Management
A. FIRST STAGE OF LABOR
1) Guard against premature rupture of membrane:
1. Keep the patient in bed, no walking allowed.
2. Avoid unnecessary P.V Examination.
3. No bearing down before full cervical dilatation.
2) Guard against Uterine Inertia:
1. Glucose 5% I.V solution
2. Sedation: relieve pain but avoid heavy sedation
3. Frequent micturition
4. Low enema to evacuate the rectum
3)Guard against infection:
1. Antibiotic: if there is prolonged labor or premature rupture of
membranes
2. Aseptic vaginal examination.
4) Observation:
1. Maternal: every 1-1/2hr).
a. Pulse, temperature, and blood pressure
b. Uterine contraction: for frequency and duration.
c. Cervical dilation and effacement
