- Head engaged intervention is indicated
a) Instrumental delivery: ventous has the advantage over the
forceps:
- Ventous occupies a smaller space.
- Ventous allows the occiput to rotate in the most suitable direction.
b) Cesarean section:
Indication:
1. Elderly primigravida.
2. Multigravida with no living children
3. Cephalopelvic disproportion
4. Failed instrumental delivery
5. Fetal or maternal distress
6. Prolapsed pulsating cord and partially dilated cervix
7. Other obstetric complications: placenta previa
C)Third Stage of Labor: Guard against:
1)Postpartum hemorrhage: by 0.04mg Methergine
2)Puerperal sepsis: By Broad spectrum antibiotic.
Complications of Occipto Posterior:
A- Maternal Complications:
1. Premature rupture of membranes and prolapse of cord.
2. Prolonged labor.
3. Obstructed labor as in case of deep transverse arrest.
4. Uterine inertia.
