HOME VISIT
Phases of home visit:
When making a home visit, community health nurse is faced with
many functions, immediate, or latent, which includes; -
I. Preparation phase (Pre-planning)
II. Introduction phase (Initiation)
III. Implementation phase
IV. Closing phase (Termination)
V. Documentation phase (post- home visit and preplanning)
Preparation phase for the visit:
Collect the data about mother /newborn need and problem.
Review the family file, records and nursing notes to understand
the family condition.
Discuss with other health team members, who are concerned with
the family health care.
Communicate with other community agencies or leaders about the
family to be visited.
Review the previously acquired data.
Identify client's need (physical, emotional, medical, developmental,
dietary, educational, and motivational).
Review previous interventions for the problems/needs identified.
Set priorities according the impact of the problem on health, client
concern regarding the problem, and ease of solution.
