HOME VISIT



               Phases of home visit:

                  When  making  a  home  visit,  community  health  nurse  is  faced  with
               many functions, immediate, or latent, which includes; -

                 I.   Preparation phase (Pre-planning)
                II.   Introduction phase (Initiation)

               III.   Implementation phase
               IV.    Closing phase (Termination)
                V.    Documentation phase (post- home visit and preplanning)





























               Preparation phase for the visit:
                     Collect the data about mother /newborn need and problem.

                     Review  the  family  file,  records  and  nursing  notes  to  understand

                      the family condition.
                     Discuss with other health team members, who are concerned with

                      the family health care.

                     Communicate with other community agencies or leaders about the
                      family to be visited.

                     Review the previously acquired data.

                     Identify client's need (physical, emotional, medical, developmental,
                      dietary, educational, and motivational).

                     Review previous interventions for the problems/needs identified.

                     Set priorities according the impact of the problem on health, client
                      concern regarding the problem, and ease of solution.
