HOME VISIT
Plan for nursing intervention according to the problems identified.
Consider equipment and supplies needed to implement the
planned interventions
Consider the available resources, and the client attitudes toward
these resources
Consult other health team professionals for certain problems if
needed.
Introduction phase:
State clearly one's name and the health facility
Explain the reason and source of referral for the visit
When entering the home, the nurse should; -
a. Like and respect for people
b. Be tactful
c. Be natural
d. Teach by demonstration
e. Never talk to other people about what goes
on in house
f. Be observant
g. Use indirect questioning
h. Choose convenient time
i. Include all family members
j. Help establish rapport with family
k. Alert to the health and welfare of all family
members
l. Able to answer question.
Implementation phase
The nurse should introduce herself to each member of the family
first then shake hands, or exchange a pleasantry in recognition of
each individual family member in group
Efforts should be made to assist all members of the to feel
comfortable and relaxed
The nurse should state the purpose of the visit clearly and ask
family members to correct or add to the information according
their individual perception
