HOME VISIT




               Documentation phase;
                     If the visit is not recorded, is not done


                     Elements  of  good  record  keeping  include  (accuracy,  conciseness,
                      legibility, promptness, and use the standard abbreviation only)
                     Accurately record the problems identified, interventions employed

                      and their effectiveness.
                     Record include future plans and recommendations for subsequent
                      home visits

                     Community health  record  is  considered  a  legal  document  and  is
                      subjected to the court order

                     The appropriate use of the patient and the family record enables
                      the  nursing  staff  to  provide  continuity  of  care  in  case  of

                      (vaccination, emergencies, and staff absences)
                     Record  is  frequently  used  as  a  supervisory  tool,  it  is  helpful  to
                      review  and  trace  the  nurse's  growth  and  development  and  to

                      determine  the  need  for  additional  help  in  working  with  the
                      families

                     When visual aides have been used during the visit, the name of the
                      pamphlet, guide or any aid should be given.


               Infection prevention and control during home visit;

                      Infections have always been a problem for health workers. As for
               infection  prevention  and  control  in  welfare  facilities  and  medical

               facilities  including  hospitals  and  clinics,  such  institutions  prepare
               infection control manuals and endeavor to adequately implement control
               measures  as  specified  in  them.  Similar  to  the  situation  in  medical  or
               welfare facilities, the first  priority  of  infection management at  home is
               the implementation of standard precautions.


                Factors affecting home infection;

                   A. Poor personal hygiene.
                   B. Infrequent hand washing.
                   C. Inadequate environmental sanitation.

                   D. Exposure to individual with infectious disease.
                   E. Exposure to stagnant water.
                   F.  Exposure to pet excreta.
