HOME VISIT



                     The nurse should talk directly to each family member of the family
                      in away that communicate that each one is valued

                     Clear question must be asked so family members expected to give
                      clear answers

                     When  the  answers  are  fuzzy  or  gray,  the  nurse  should  seek
                      clarification until the answer be clear

                     The nurse always be a model of confidence

                     The nurse should validate the accuracy of the initial assessments

                     Collect additional data the family

                     Identify any new problems that arise

                     Work at mutual goal setting for problem solution with client and
                      family

                     Restructure priorities based on new data

                     Implement nursing interventions as planned or modify the plan as
                      the situation dictates

                     Use the home facilities as possible

                     Refer to the community resources if needed

                     Evaluate the effectiveness of the nursing interventions in term of
                      the client's response.


               Closing phase;

                     The nurse terminates the visit with brief review of the important
                      points

                     She stresses the positive aspects, and emphasizing family strength

                     She repeats the plans the client, family will carry out in her absence
                     Together the family and the nurse plan on the next visit


                     Establishing a date and approximate time convenient for the nurse
                      and family.
