extreme pain). Mr Charles further explained he is independent with his activities of daily
livings and his pre injury domestic chores. Mr Charles reported he participates in
physiotherapy intervention once per week and hydrotherapy intervention twice per week
with reported benefit. He noted that the treatment consists of massages and TENS
machine. Mr Charles advised that upon completion of hydrotherapy and physiotherapy he
goes home and lies down for 3 hours.
Mr Charles reported he is scheduled for MRI scan on the 14th of August 2003. He requested
to change nominated treating doctor. We advised Mr Charles that he require to submit the
request in writing to the insurer. We discussed return to work plan/ program with Mr
Charles. Mr Charles agreed to the return to work program developed during the meeting
provided his NTD allowed him to return to work.
Employer
We have been in regular contact with Mr Main (employer’s representative) since the 7th of
August 2003. Mr Main is aware of Mr Charles’s current progress and return to work
development. Mr Main indicated that due to the size of the business, there are no suitable
duties availably. We explained financial incentives for provision of suitable duties to Mr
Main. Mr Main was unable to provide suitable duties. He agreed to provide pre injury
duties on reduced hours. Mr Main agreed to the return to work plan developed during the
meeting.
Nominated Treating Doctor
We attempted liaison with the Dr (nominated treating doctor) on the 13th of August 2003
via verbal correspondence, with nil success. We sent written correspondence to Dr on the
15th of August 2003. Mr Charles advised that Dr Main had recommended bed rest if his
injury did not improve.
