Reporting Process – (MySafety)



  It is Co-op Property’s policy to ensure that all accidents, incidents, hazards and
  near misses are reported. These can then be investigated as required. MySafety is
  the system that Co-op expects the supply chain to use to log all reports. To access
  MySafety go to https://coop.uk/mysafety

  You can access on to MySafety from a desktop PC, a laptop, a tablet or a
  smartphone.

  All accidents and incidents are required to be reported within three hours of
  occurrence, using the incident form on MySafety. There is no time requirement for
  a near miss submission.

  The Property RHSE Team will use the information provided to complete
  investigations and look for trends in the data.

  All visits from enforcing authorities that did not culminate with enforcement notices
  or Fees For Intervention (FFI) need to be reported by email to
  RHSEProperty@coop.co.uk within 3 hours. This will then be logged on MySafety.
