             4.2.3.15.   Any person who loses his/her borrowed books may settle through the following;
             4.2.3.15.1.   Replace with the same title plus prescribed processing fee.
             4.2.3.15.2.   Pay its current replacement value plus prescribed processing fee.
             4.2.3.15.3.   Replace of different title as per approval of the librarian.

             4.2.3.16.   Failure to return a circulation book. Any person who fails to return any book on its due date
                       shall pay a fine of 1.00 per hour/per book. Non school days are not included in the computation
                       of overdue fines.
             4.2.3.17.   Borrowers will be held responsible for any mutilation found in library materials when returned.
                       They must check and report any mutilation found before borrowing.

             4.2.4.  Library Card Application
             4.2.4.1.   Present current enrolment form to the librarian in-charge.
             4.2.4.2.   Submit one (1) 1x1 ID picture.
             4.2.4.3.   Fill up the application forms.
             4.2.4.4.   Come back on the date specified and claim your borrower’s card by presenting your School ID.
             4.2.4.5.   Application of new LBC and validation of old LBC is done anytime during library hours.
             4.2.4.6.   A prescribed fee shall be charged for duplicate of lost Library Borrowers Card.

             4.3.   Health Services
             4.3.1.  Goals and Objectives
             4.3.1.1.   To provide primary medical care to students, faculty and personnel and the community.
             4.3.1.2.   To ensure a healthy working environment for the students, employees and faculty members.

             4.3.2.  Services
             4.3.2.1.   Handles simple, uncomplicated and stable medical cases.
             4.3.2.2.   Engages in preventive-promotive activities such as physical, medical and dental examinations
             4.3.2.3.   Renders diagnostic and treatment services.
             4.3.2.4.   Attends to accidents and emergencies
             4.3.2.5.   Refers cases to hospitals.
             4.3.2.6.   Provides assistance in the screening of prospective freshmen students.
             4.3.2.7.   Keeps medical records of students and employees.

             4.4.   Food Services
             4.4.1.  Goals and Objectives
             4.4.1.1.   To provide nutritious and reasonably priced food to the students, employees, faculty, officials
                       and guests.
             4.4.1.2.   To ensure a variety of food available for breakfast, lunch, dinner and snacks for the students
                       and employees.

             4.4.2.  Services
                       4.4.2.1. Ensures  a  variety  of  food  services  for  breakfast,  lunch,  snacks  and  dinner  for  the
                               students and employees
                       4.4.2.2. Provides catering services through the school canteen and the various food stalls within
                               the university.

             4.5.   National Service Training Program (NSTP)
                           The  National  Service  Training  Program  (NSTP)  Act  of  2001  requires  all  Filipino  male  and
                    female  students  in  undergraduate  courses  to  undergo  6  units  of  NSTP  courses  as  graduation
                    requirement.
                           IFSU implements two (2) NSTP components, the Reserved Officers Training Corps and the
                    Civic  Welfare  Training  Service.  The  NSTP  has  two  parts;  NSTP  1  should  be  taken  during  first
