BASIC LIFE & VOLUNTARY LIFE INSURANCE PLANS

Employees must enroll in employee coverage for dependents to be eligible for this benefit.
Your spouse’s rate is based on your age. The premium amount is found in the box where the row (your age) and the column (benefit amount)
intersect. Providing you are still employed, your benefits will reduce to 50% at age 70 and your Spouse’s coverage ends at the age of 70.
