GROUP CRITICAL ILLNESS INSURANCE COVERAGE
INSURED BY CIGNA
Critical Illness insurance coverage provides a cash benefit to the policyholder when an
insured person has a covered illness event. Benefits include coverage for heart attacks, stroke,
invasive cancer. Available for you, your spouse and your children. Wellness Benefits are also
included for each member. Additional benefits beyond option 1 and 2 can be accessed online
in the employee navigator.

ELIGIBILITY
All Active Full Time Employees working a minimum of 20 hours per week.
HOSPITAL CARE INSURANCE COVERAGE
INSURED BY CIGNA
Hospital Care insurance coverage provides a cash benefit to the policy holder when an insured
person incurs a hospital stay. Benefits are separate from your medical plan, and are paid
directly to you.

All Active Full Time Employees working a minimum of 20 hours per week.
