Please update your DDS personnel record. Log on to TeamGeorgia, www.teamgeorgia.gov; go to
Employee Self Service, Personal Information, and Update. It is very important to have your correct
address in the system so that you can receive benefit information, on a timely basis, from the State
Health Benefit Plan (SHBP), providers and Human Resources.
