Population health data will be utilized to pinpoint areas for
service development, while service delivery data will highlight
opportunities for improvement. All services will undergo evaluation
to ensure that patients achieve meaningful outcomes and that
harm is prevented. Additionally, this approach will guarantee that
the NHS delivers optimal value for taxpayers, reduces medication
waste, and recognizes the importance of quality care. Community
pharmacies will leverage a shared data platform, ensuring easy
access to information that facilitates enhancements in service
delivery and improves patient outcomes.
5 The model can be flexible and adapted to meet
the needs of each of the countries across the UK,
recognising the differences, and that different
elements will be implemented within varying
timescales in each health system.
Given the varying commissioning priorities in each country,
the model must be flexible and responsive to accommodate
these differences. Each health system will clearly
understand how the future model of pharmacy practice
aligns with the specific population health needs in their
area.
6 Pharmacists will fully utilise their clinical
expertise, primarily prescribing, to improve the
health of their patients.
Pharmacists will consistently and comprehensively
oversee repeat prescribing and supply, assisting
individuals with long-term conditions. They will
also provide support for patients dealing with
acute, common, and seasonal illnesses, as well
as engage in other public health initiatives that
involve prescribing. Additionally, pharmacists will
leverage their expertise to prescribe personalized
medication solutions, utilizing advancements in
pharmacogenomics.
