               (2) There is a defined pattern (three or more occurrences) of calling in sick on consecutive
               Mondays, Fridays, or days before/after a holiday.

               FMLA and Worker's Compensation are not counted as occurrences under this policy.

               Doctor, dentist, and therapy appointments are not counted as occurrences if the supervisor is
               notified at least one day in advance of the appointment; however, if the number of
               appointments appears to be excessive, the supervisor may require a medical certificate.  With
               the supervisor’s permission, an employee may be allowed to make up the missed work time for
               appointments in lieu of using sick leave.
