COMMUNITY HEALTH INFORMATION TRACKING SYSTEM (CHITS):
AN EVALUATION

The study was designed to assess the Community Health Information Tracking System
(CHITS) of Moncada, Tarlac. The descriptive method of research with the questionnaire as the
main data gathering tool was used to answer the said problems The locale of the study was in
Moncada, Tarlac with the Rural Health Unit I and II health care providers as the respondents.
Problem Number 1 dealt on the present operational status of the community health information
tracking systems (CHITS) in terms of: a) organizational structure and management and b)
scope and operation. Problem Number 2 focused on the level of effectiveness of the community
health information tracking systems (CHITS) in terms of accuracy, reliability and functionality.
Finally, Problem Number 3 dealt on the plan of action to be developed to improve the present
community health information tracking system of Moncada Tarlac. Frequency counts, percentage
computation, mean and average point values were used as tools for data analysis.
The following are the salient findings: 1) Midwives dominated the number of
respondents with frequency result of 16 or 53.3 percent, and in terms of the ratio of health
workers to community, the indicators “3 barangays” and “20 barangays” have the highest
number of frequency of 10 or 33.3 percent while the lowest percentage is 3.3 percent and both
handles four (4) and one (1) barangays: 2) In terms of Scope and Operation, in the category
of: functions of health worker, as the usual tasks of the health workers to “admits and records
information of the patients”, “attends to the needs of the patient during their visit”, “facilitates
record and report” have weighted mean 4.57, 4.87, and 4.70 as well and have descriptive
equivalents “always”. In like manner, the two indicators “makes accomplishment report” and
“reminds patient with their follow up schedule” both have descriptive equivalent “often”; 3)
On the basis of record management, only two items got the result of descriptive equivalent
“always” with weighted mean 4.50 and 4.57 respectively. In terms of delivering health services,
there were two items got 4.53 and 4.57 percent respectively, both with descriptive equivalent
“always”; 4) On the level of effectiveness of the Community Health Information Tracking System
(CHITS) in terms of accuracy, the results show “effective” with average weighted mean 4.21;
5) On the level of effectiveness of the Community Health Information Tracking System (CHITS)
in terms of functionality, the results show “effective” with average weighted mean of 4.03; 6)
On the level of effectiveness of the Community Health Information Tracking System (CHITS) in
terms of reliability, the results show “effective” with average weighted mean of 4.33. However,
on the overall weighted mean in terms of accuracy, reliability, and functionality, it is ‘effective”.
In the light of findings, the researcher concluded the following: 1) The health care
provider-respondents were midwives dominated, and mostly handled 3 and 20 barangays in
terms of ratio of health workers to community; 2) In terms of scope and operation, the CHITS
serves as an easy access to the health workers in attending to the needs of the patients and
have easily access in recording patients’ data; 3) On the level of effectiveness of CHITS in terms
