investigated, and will be kept confidential to the maximum extent possible. We prohibit any
form of retaliation against an employee for filing a bona-fide complaint under this policy or for
assisting in a complaint investigation.
There is no requirement to follow any formal chain of command when filing a complaint. You
may file a complaint or express any issue of concern with the Human Resource Service Team, or
any member of the Executive Team at 1-888-222-0457.
As an employer, we are subject to certain government regulations, including recordkeeping and
reporting requirements for the administration of civil rights laws and regulations. In order to
comply with these laws, we invite our employees to self-identify their race and ethnicity.
Submission of this information is voluntary and no adverse treatment will occur because of a
refusal to provide it. The information will be kept confidential and will only be used in
accordance with the provisions of applicable laws, executive orders and regulations, including
those that require the information to be summarized and reported to the federal government
for civil rights enforcement. This information is maintained separate from your employment
file. When information is reported, data will not identify any specific individual but as an
aggregate.
This policy acknowledges diversity in the workforce and the possibility of conflict and provides
open access to all levels of management in an attempt to rectify problems. It is our objective to
ensure that all employees work in an environment free from any form of discrimination,
including harassment. Any incident or situation that is believed to involve discrimination or
