Workstation Assessment Procedure
Linked to: Safety and Wellbeing Policy

2 Emails safety.feedback@delwp.vic.gov.au and requests assistance Manager
from a Safety Advisor or Wellbeing Team Member.
3 • Reviews the self-assessment with the employee and manager • Safety Advisor
and meets to review at the work station if required.
• Wellbeing Team Member
• Provides advice on the suitability and adjustment of current
equipment and make recommendations for changes if:
o Workstation adjustments and chair needs are standard
departmental requirements.
Refer : Workstations and Chairs Guideline.
o If minor equipment needs (eg telephone headset or foot
stool) are required they may be purchased through the
departments stationary provider.
• Contacts the Injury Management Advisor if an issue with
injury, illness or discomfort is raised
• Consults the manager about obtaining the services of an
occupational rehabilitation provider when relevant.
• Hands over the management of case and injury support to
Injury Management Advisor.

Note: Skip to step 10 if an Occupational Rehabilitation Provider
referral is not required
4 Provides a copy of the Request for Workstation Assessment form Wellbeing Team Member
to the manager and discusses the costs of the assessment.
5 Completes the referral form and returns it to the Wellbeing Team Manager
Member.
6 • Contacts an occupational rehabilitation provider and explains Wellbeing Team Member
the referral including expectations of the assessment, report
and costs including travel.
• Sends the referral to the occupational rehabilitation provider.
7 Arranges a time to complete the assessment with the • Employee
occupational rehabilitation provider. • Manager
8 • Reviews the recommendations in the report provided by the • Employee
occupational rehabilitation provider. • Manager
• Contacts the Wellbeing Team Member if there are queries
regarding the recommendations.
9 • Reviews the report provided by the occupational • Wellbeing Team Member
rehabilitation provider. • Manager
• Places a copy of final workstation assessment report on
employee personnel file.
