Workstation Assessment Procedure




               Linked to: Safety and Wellbeing Policy

                       o  arranging with the manager for a referral to an
                           occupational rehabilitation provider to conduct the
                           assessment.

             5      Completes the Request for Workstation Assessment form if     Manager
                    required and provides it to the Injury Management Advisor.
             6      •  Contacts an occupational rehabilitation provider if required   Injury Management Advisor
                       and explains the referral  including expectations of the
                       assessment, report and costs including travel.
                    •  Sends a referral to the occupational rehabilitation provider.

             7      Arranges a time to complete the assessment with the          Manager
                    occupational rehabilitation provider or Injury Management
                    Advisor.

             8      Participates in the assessment with the Injury Management    Employee
                    Advisor or the occupational rehabilitation provider.
             9      •  Adopts  the recommendations in the report provided by the   •  Employee

                       queries about the recommendations.

             10     Implements any recommended work practice changes and         •  Employee

                    Note:

                    •  All equipment costs are met by the business unit.
                    •  Medical information may be required if equipment
                       recommended is non-standard and of significant cost. Contact
                       the Injury Management Advisor for advice.

             12     •  Obtains authority from the employee to request medical    Injury Management Advisor
                       information from the medical practitioner.

                    •  Requests information from the medical practitioner about the
                       need for the non-standard equipment based on the nature,
                       severity and prognosis of the medical condition of the
                       employee.
                    •  Reviews the reasonableness of medical advice received from
                       the medical practitioner with the manager.
