Paid Family Leave


                   Employees may be eligible for partial wage replacement benefits
                   under the Paid Family Leave Act for up to a maximum of six weeks

                   for the following reasons:

                   •  To  bond  with  a  new  child  after  birth  or  placement  for

                        adoption or foster care;
                   •  To care for a serious health condition of an employee's child,

                        parent,  parent‐in‐law,  grandparent,  grandchild,  sibling,
                        spouse, or registered domestic partner.


                   The Paid Family Leave Act provides benefits based on past quarter

                   earnings for up to six weeks in a 12‐month period. The cost of the
                   insurance  is  fully  paid  by  the  employee.  The  12‐month  period
                   begins on the first day an employee submits a claim.


                   To be eligible for benefits, employees may be required to provide

                   medical and/or other information that supports a claim for time
                   off to bond with a new child or to care for a child, parent, spouse

                   or registered domestic partner with a serious health condition.


                   The employee is responsible for filing their claim for family leave
                   insurance benefits and other forms promptly and accurately with
                   the Employment Development Department. A claim form may be

                   obtained  from  the  Employment  Development  Department  by
                   telephone,  letter,  the  Internet  or  in  person.  All  eligibility  and

                   benefit  determinations  are  made  by  the  Employment
                   Development Department.
