7.  The next page of the E-MNR Submission will include a number of fields pertaining to the
                       patient’s status. The therapist should periodically save their progress in a draft as fields
                       are completed by clicking Save in the top right of the E-MNR form.










                          a.  Once saved successfully, a highlighted alert will appear to confirm.



                   8.  After all required fields (marked with R) have been completed, click Next at the bottom of
                       the form.
                   9.  Click Review Before Submitting to confirm all required information has been input.













                          a.  If there are any errors on the E-MNR that must be corrected before submitting,
                              they will be listed by section and description.















                   10. After the review is complete and necessary changes have been made, click Submit.
                   11. The confirmation page will show the E-MNR # and Submit Date and time in the top right
                       of the page. Print or download/save the page and upload it to the patient’s eDocs in
                       WebPT as “ASH E-MNR Submission #(E-MNR #)”
                   12. Update the appropriate clinic’s Authorization Log in Google Drive with the
                       Authorization (E-MNR) # and Date Submitted.

               To check the status of an E-MNR submission through the ASH portal:
                   1.  Click Search in the blue toolbar on the home page.
