9. In the Questionnaire(s) box, select Yes as the response to Can this service be
performed in a freestanding In-Network facility?
10. Attachments (clinical notes) are not necessary to include.
11. The Comments box can be used to request retro-authorization if the submission was
made outside of the 3-day window. Indicate the requested backdated start date.
12. Confirm Contact Information fields. Click Next.
13. Review all of the request details before clicking Submit.
