7. After all requested changes have been made, click Submit at the bottom of the page.
8. Clinical notes (initial evaluation, progress notes, case notes) must be uploaded in the
Tricare portal to be reviewed with the referral/authorization request. *The patient’s
PCM will not approve any requests without supporting documentation!*
a. Download each individual note from the patient’s WebPT chart.
b. In the Tricare portal, select Document Type: Clinical Notes Supporting Medical
Necessity.
c. Browse for and Attach each file. After all of the files have been attached, close
the window as the request with documentation is complete.
9. Add a Chart Note detailing when the referral/authorization request was submitted, the
number of visits requested, and what clinical notes were included. (i.e. “Submitted
request through Tricare portal for additional 12 visits for referral/auth #0309-181210-
11445 & attached the following documents: Script 4.2.19, Case Note 4.2.19, Progress
Note 3.8.19, Progress Note 1.18.19, Initial Eval 12.13.18”)
10. Update the appropriate clinic’s Authorization Log in Google Drive with the
Authorization # (referral #) and Date Submitted.
Note: From the patient’s Referral Details page, you will be able to view all files that have been
attached by clicking Show + next to Attachments and Other Documentation at the bottom of
the page.
This will expand the section and list all attachments with Date uploaded, Document Type, and
File Name information.
